Role Overview:
As a Community Health Worker, you will be responsible for providing support and assistance to individuals and families to help them access healthcare services and improve their overall health and well-being. You will work closely with community members to address their health needs, provide education, and connect them with necessary resources and services.
Key Responsibilities:
- Conduct home visits and community outreach to assess the health needs of individuals and families.
- Provide information and guidance on accessing healthcare services, including appointments, medications, and treatment plans.
- Assist clients in navigating the healthcare system and connecting them with community resources and support services.
- Develop and implement health education programs and workshops on topics such as nutrition, wellness, and disease prevention.
- Monitor and follow up with clients to ensure they are receiving appropriate care and support.
- Advocate for clients and help them address barriers to accessing healthcare and social services.
- Maintain accurate and confidential client records, including health assessments, service plans, and progress notes.
- Collaborate with healthcare professionals, social workers, and community organizations to coordinate care and support for clients.
- Participate in community events and outreach activities to promote health awareness and engagement.
- Stay current with developments in community health and best practices for client support.
Qualifications:
- Diploma or degree in Community Health, Social Work, Nursing, or a related field.
- 1-3 years of experience in a community health or support role is preferred.
- Knowledge of healthcare systems, community resources, and social services.
- Strong interpersonal and communication skills, with the ability to build relationships and provide support to diverse populations.
- Compassionate and empathetic approach to client care and advocacy.
- Ability to work independently and manage a caseload effectively.
- Experience with case management and health education is an asset.
- Bilingual proficiency in English and another language is a plus.
